Trying to add a third party component

Hi, I'm trying to simulate this circuit but I need to use the ad629 for it to work. I have tried to use the .lib and .inc but has not work for me. http://www.analog.com/media/en/technical-documentation/data-sheets/AD629.pdf for datasheet and for the models http://www.analog.com/en/design-center/simulation-models/spice-models.html

I'm new to this so I would appreciate if someone can tell me what I'm doing wrong or why is it not working. Thanks!
