Damaged fibre optic card

Hi,
This is a QLE2460 fibre optic PCB from Windows server...
Question is: what could cause the damage, it is hapend with new cards every 5-6 mounts?

On a server, usually too much heat. Sometimes, too much vibration.

Even if your operating conditions are within specification, life can be extended by running it at lower temperature.

Also, some brands/chip makers put more effort into high temperature lifetime. If you are having trouble with a low-cost or generic board, buy a more reputable brand. If you are even having trouble with a reputable brand, but something made more specifically made for more difficult operating conditions (high temp, high vibration, ...).

Other issues could be that the card is fine but the connector is bad/sporadic. Or, power surges to the unit. You may want to double check power line conditioning to your server.

Temperature in the room is air conditioned, 18-20 Celzius, that also happens on PCB of Hard Disks...
Fibre optic is usually Dell or HP... Is there any possibility that happens because of X-Ray radiation from
Computed tomography (that server do)?

Yes. Kicking a bit off of a doped semiconductor is possible and a concern for X-ray inspection of components.
