On a server, usually too much heat. Sometimes, too much vibration.
Even if your operating conditions are within specification, life can be extended by running it at lower temperature.
Also, some brands/chip makers put more effort into high temperature lifetime. If you are having trouble with a low-cost or generic board, buy a more reputable brand. If you are even having trouble with a reputable brand, but something made more specifically made for more difficult operating conditions (high temp, high vibration, ...).
Other issues could be that the card is fine but the connector is bad/sporadic. Or, power surges to the unit. You may want to double check power line conditioning to your server.

Yes. Kicking a bit off of a doped semiconductor is possible and a concern for X-ray inspection of components.
