I'm having trouble finding a tric coupled photo with two independent outputs. Diode input can be the same, it is not necessary if 2 separate diodes. Someone could indicate a specific model, website or name of the component.

Vishay has quite a selection of optocouplers:

You can put the two input diodes in series if you can't find a coupler with only one input.

 

I've never seen one. What's wrong with two single ones? There's loads of those to choose from.
