Induction Motor Connection- Help Needed

I bought this motor from Amazon and am finding it difficult to make the connections Please I need help.
The motor did not come with Capacitor, I was planing to use 10uf 400v Capacitor with it but dont know how where the capacitor connections goes and direct connection.

The D162 does not require you to add a capacitor for starting: http://www.swdrives.com/HVAC Motors.pdf
If you are in the US, you would attach the black (or any non-green, non-white) lead to terminal 4 (call it L1). The white goes to terminal 1 (L2), and the green goes to one of the case screws.

Directions of CW rotation are given in the pdf. You reverse the yellow and blue linesin the motor.

Is that plastic piece with the push-on connectors broken? Has the motor been used?
