Flyback Transformer design Pt2

Hello All!

I'm in process of troubleshooting a Fly-back controller/transformer design and I wanted some more experienced eyes to help if possible.

Right now, I'm having issues with the output voltage of the transformer, it supposed to output 24VDC but when i put different combination of load resistors it outputs at 1K ohm: 5VDC , 500 ohm: 3VDC, or 333 ohm: 2VDC. To me it seems like the Feedback coil on the transformer is sensing incorrect voltage.

I initially had a mistake where I connected the output of the transformer backwards. I have fixed that and also added a diode there aw well. This fix allowed the transformer to output some voltage. (Not the correct voltage)

I have attached pictures of the PCB, SCH, Gate scope, and FB scope (From the fly back controller IC).

The FB voltage is very high i've noticed as well.

Does anyone notice anything else that looks out of place?
