That sure looks like the ST Microelectronics logo but I was unable to find an opamp with either of those numbers on it. Could be a proprietary number for a customer's specification rather than a catalog part number.

I don't think it is a common quad op-amp. The package is too wide. If it is a quad op-amp then it might be a power version.

Have you verified that the part has the power and ground on the expected pins?

My guess is that it is not a quad op-amp but, instead, a more specialized part. What is the part in? Knowing that would help get a better answer.

 

This is found in all of the Myo armband pods. Each pod has a set of three electrodes to sense muscle electrical activity.

I figured this was either a op-amp package or instrumentation amp.

Most likely - but I'd see if "PHL 423" finds anything before giving up.
