Arduino 12v - 40v + Rife/PEMF design change

I am a rookie (not totally, lots of circuit bending and basic repair work), with lots of coding knowledge. Would anyone be so kind to help me find a simple way to make this design go over 20v without interfering with the output frequency from the probe.

I wanted a good project to learn more and had remarkable success with rife machine on my busted ass shoulder, I have great results at 20v, I want to buy one of the newer 40v devices but figured I could make it. And ya i know that packs a punch but destroyed tissue don't open up a blockage as quick on low voltage. So ya, I know that FutherMucker packs a punch. :) I like steak well done...

I think I know what to do... here is the design. Could anyone give me some pointers?

I don't see how your present design gives you a 20V pulse from a +5V supply, is the circuit image complete.?

Excuse me, I was not clear enough.

He says it does not exceed 12v. I should have been more clear as I meant to go up from the 9-12v design > (to) 40v ish. I looked at boost modules that did not have a crazy high voltage but was around that range for safety reasons.
