It's completely find and good to spend months and years looking at all of the details and what could have been done differently and what should be done differently going forward.

But one thing that gets lost in most of these inquiries is the fact that at the end of the day these "500 core participants" will arrive at a conclusion, after months of testimony and consultation and research and testing, of what was done wrong and then people will demand that those that didn't do it right be held accountable for not doing so. That's a horrible bar to set. These "core participants" should be given the same amount of time (in this case about two hours), under similar circumstances and with the same amount of knowledge and with the same stress and pressure associated with the overall situation they were facing, to conclude definitively that the actions taken by those on the scene that had to actually make the decision at the time were wrong.

Apply what you just said to the destroyer collisions. It's the best bar. Because it expects the highest standard. Being able to see the future. Or I should say........be able to see the correct future.

The bar is not meant to condemn people. It judges competence of procedure, not personalities. Some entities will not change procedure unless a death or even multiple deaths are involved. The reason for this is not because we have fuddy duddies running things. It because the current procedure is made from previous deaths.

People need to be reminded that the current procedures came from previous deaths.

We all saw how the navy applied this bar. Let's see what the London fire/emergencies dept does with it.

But I certainly agree with you on the political/personal abuse of such committees/studies.

errrr... nope ... I'm happy to say that you're quite right about that ... that's a feeling that I will most definitely never get to know ... My parents were wonderful people, the biggest blessing that my life's ever had. My mom's still around (she's 85), but I lost my dad when I was 27, the same year that I got married.... boy, I miss the old man...

The Mexican Spanish equivalent of the expression "gross!" is the (very informal) word "guácala!" ... Indeed, it has an onomatopoeic origin: the word itself resembles the sound one makes when one is reflexively trying to gag.

Now that you've all been informed, I can't think of a better example on the proper use of the word "guácala!":

If you need to be advised of this, then where have you been living?

This from a country that advise visitors/tourists that if going on a 'walk about' carry a stick and do NOT stick your hand in any hole in the ground!!.

I doubt there's a single human being in this planet older than 5 years old that would need to be advised against getting close to crocs...

So, if you are ever called upon to make a split-second decision, you are saying that you are completely willing to let your actions be judged based on months and months of dozens, if not hundreds, of people researching, analyzing, and debating all of the factors that played a role in the consequences of your decision, including those that were unknown to you and of which you had no reasonable way to be aware, and taking into account vanishingly low-probability consequences that happened none the less, and determine, based on all of that, that you made the wrong decision in the half second available to you to assess the situation, evaluate possible actions, decide on a course of action, and take it (i.e., far less time than it took me to write just this sentence -- or about 1% of the time it took you to read it).

And yet every year that are numerous injuries and sometimes deaths associated with people intentionally trying to get near crocs and gators, usually to get that "selfie to die for".
