I have got this DIY bench grinder, have tried to balance the disk but unsucessful, Any idea to balance the disk to remove vibration?

We were all hoping that You would create your own Pictures or Video, not use someone else's.

Pictures can be directly added to your next Post as an Attachment.

Video must be up-loaded to YouTube first,
then add a Link to your Video as a Copy & Paste-HTML-Address in your next Post.

A Bench-Grinder is a very useful Tool for any Shop,
They are very inexpensive.
I would never recommend that someone should try to construct one out of used parts.

This is completely unacceptable.
A sleeve must be Machined to create a tight-fit
between the Motor-Shaft and the Hole in the center of the Grinding-Wheel.

Cutting the Shaft as short as possible is also a requirement.
Large Washers must also be installed to prevent any "wobbling" from side to side.

Buy a commercial Grinder,
they are cheaper than making what You have work acceptably well.
