I posted all you need to make your cord
One showed the wire one shows the plug
CED can get you the supply's the cords not going to be cheap.
You may google and find something cheaper but
Then its sure to be made of low Quality

 

How do you handle lock-out, tag out?

I would fit a disconnect as per the manuf DWG.shown.
They show a RCCB but a fused or non-fused breaker would also work, and have the ability to lock-out.
