You can use other opamp. If you understand how my circuit works, then you can simplify it at high voltages.

the signals to each inputs are differential signal, does your signal source represent differential signal, 4 volt peak to peak centered at 2.5V?

 

No.
input 2V peak to peak or ampl.=1V
output=gain*input ampl.=0.747V or1.493V peak to peak
From my first picture
out_ampl: PP(v(out)/2)=0.746572 FROM 0 TO 0.003
gain: out_ampl/1=0.746572

And yes, this amplifier does not amplify the signal. You will see the transmission ratio of your prototype. I just adapted the circuit to a unipolar power supply.
