Neebie! Can someone help?

I was wondering if someone can map this out for me on paper, I believe that chip is some sort of relay or possible 555 timer? Buts it's 6 pins and I'm not sure how to look these components up. I'm basically trying to build my own version of this circuit on a breadboard

I know that the bottom 3 solder points are(from left to right) ground then V input then output

A little more context might help find the answer. What is the assembly and where did it come from? What is it supposed to do?

Hello,

It looks like the PCB is double sided, as I see some vias.
Also there is a little yellow spot on the 6 pin chip covering the text.
The text might reveal the device data.
