From its appearance I'm guessing it's an opto-isolator.
But don't keep us guessing. It would help if we knew make and model of whatever unit it's part of.
Do you know what caused it to fry?

A power surge caused it to fry. The building lost power and when the controller restarted, it stopped working.

It’s a Siemens PXC 128 U.

Is it a generic component that can easily be found online or ordered or we need additional details to find which one to order?

 

Looks like CM04RC01T from Taiyo Yuden. A common mode choke that has been discontinued.
CM04RC matches the packaging, 01T matches the marking.

I have never ordered from these places. All the big places have none.
