Hello everyone!
I have this old board here that I am trying to identify. It has an elco connector labeled 6060897 (7022). The board itself is labeled SEL722A (8522-1). See photographs. The board consists of four Fairchild 91429. The original packaging was labeled Fairchild icu914. There were some old faded schematics with the unit for designing various projects such as wide-range audiogenerator, 1khz wein-bridge sine, DC meter amplifier, shmitt trigger and sine-square converter. The paperwork itself was hand drawn with what appears to be blueprinting inc and it is labeled fairchild icuL914.
