There is one here for the MC2100 series, I assume you would want the PWM version? , there is also the older Triac controlled ver also.

I have a JC22-001 controller board with no DC output going to the motor. I sent the board out for repair and I am still getting the same results. This board is on a Horizon LS760T treadmill. The only thing that does not work is DC power to the motor. Does anyone have a schematic or knows what enable the motor to get its DC power
