I made a signal amplifier which amplify from the 0-5V rectangular wave to -5-+5V rectangular wave. The schematic and simulation are attached.

But after I test the board. The output are not what I expect. Below are the input and output wave.

Check voltage on pin 11 should be minus 5 volts. Schematic shows + voltage on pin 11.

The pin 11 in the first schematic and V- in the second schematic are checked, -5V.

 

Besides the schematic did you verify the -5 volts in the circuit?
If you look at the first schematic it shows a battery + going to pin 11

Yes, I measure the pin V- on the board which is -5V.

With R85 to ground instead of 5 volts I'm looking at a non inverting amp with a gain of 2.

In that case, the output would (try) to go between 0V and +10V but the amplifier has only a +5V supply.

The LT1639 has specialized characteristics. You can find reviews about it's different applications.

We find applications using single supply 5V however low frequency tiny signals uses 3V THD best @ 2.6V or less.
Some are quite different from garden variety op amps.

The frequency is from 5 to 500Hz

I use LT1638. I used LT1639 when I simulate the circuit but LT1639 was out of stock.

Who said anything about +10 volt?

With you revised R85 connection, a X2 non-inverting amplifier, and a 5V input, what will the output voltage be if not +10V?
Impossible with a positive supply of just +5V.

 

To get back to the original problem, What is the input frequency which produces the output waveform shown in post #1?

 

Seems to me like you are making a meal of it.
