Happy new year everybody!

I made a signal amplifier which amplify from the 0-5V rectangular wave to -5-+5V rectangular wave. The schematic and simulation are attached.

But after I test the board. The output are not what I expect. Below are the input and output wave.

Besides the schematic did you verify the -5 volts in the circuit?
If you look at the first schematic it shows a battery + going to pin 11

The LT1639 has specialized characteristics. You can find reviews about it's different applications.

In the same category LT1490 which is related.
We find applications using single supply 5V however low frequency tiny signals uses 3V THD best @ 2.6V or less.
Some are quite different from garden variety op amps.
