Phase shifted full bridge converter design

I am designing a phase shifted full bridge (PSFBC) dc dc converter. My input is 350V and output is 60V. But my PSFBC is working only on above 90% duty cycle. Below 90% duty cycle the converter is unable to provide current to the load.
Can you people please let me know where i have done wrong
