You take 5 VDC and convert it to AC, run it through a transformer and then rectify it back to DC. Your question, with no mention of current requirement or application is too broad or open ended to provide a good answer so you get a generic answer.

You would do well to just use a 12 VDC or 24 VDC to 220 VAC Inverter and recetify the 220 VAC out of the inverter.

Additionally 200 VDC is not considered High Voltage, depending on location and the applied standard.

It is powered by usb cable and current requirment is negligible
Main focus is small size

Then I would be looking to use a camera flash circuit as tcmtech suggested. That or build a small inverter. A Google of Inverter Circuits will get you few basic designs which should work. Use a small 220 volt to 6.3 volt transformer in reverse for a transformer.
