Details follow:

Apparently I violated Tony's image numbering convention:

For images in the Reference Volume 5 , I was Using Volume 1 numbers which were already in use. This works in html at ibiblio or the PDF because the images are in separate subdirectories, where as AAC puts them all in one subdirectory.

The fix was for me to convert some images I had added to volume 5 to comply with Tony's numbering convention. (Tony does re-use some images. For example, a few in Vol 5 Ref have been copied from Vol 1 DC. This works if the images in both subdirectories are identical.)

Volume 5 equations.sml contrib.sml changed. Images that need to be reloaded from ibiblio to AAc include:

Volume 5, Ref: 1109[5-9].png 1110[02].png
Volume 2, DC: 1023[12389].png 10241.png

It may be better to re-load all images, as they get corrections from time to time.
