The summing block with two "plus" inputs represents the sum:

\(V_4(s) + \frac{V_4(s)}{G_2(s)}\)


So you can replace that with a single block having the indicated transfer function and an input of \(V_4(s)\)

Two cascaded blocks represents the multiplication of the two transfer functions.
