Trying to create a 4-16 bit decoder with 4-bit input, 16 output signals. I think this is correct, can anyone verify:

 

Do you want the outputs to be active-HI or active-LO?

What steps have you taken to verify the correctness of your design?

Derive the functionality (equations or truth table or whatever) from the diagram and compare that to the function the circuit is supposed to implement.
