An on/off switch, a huge inductor, a diode and a capacitor.

Power Integrations has a couple of lines of off line switching power supply chips that include output transistor on the chip. Their topologies result in very low parts count designs, which are well documented in their application notes.

Why do you wish to design such a power supply when you can buy ready-made power supplies that are already certified safe and exhaustively tested for reliability and functionality?
