Linear Actuator Circuit Part

Hi, a while ago I had a friend who created this linear circuit diagram for me but I don't know the components I need to get. It is to control a 4-6v DC motor that powers a jack screw linear actuator. It has adjustable limits that stop the motor when the nut travels to either end. When the switch is pushed/flipped, the nut then travels to the other end and stops. It is controlled by a 2 position switch. Can someone help me as to what components I need to buy? Thanx.

Yes, I think that is it. So, all I need to get are some diodes and switches? Can you tell me what diodes I need to get for the following voltages....and also, do you know when I can get the very small limiter switches? This is for hobby purposes.

3.7 volts
7.4 volts
12 volts
