If you hit the live cell with 40000 volts in 1 millisecond, once, like a sting, the cells will live and nothing will happen to it. Unless if you keep the plug on then yes you will fry the cell

You base this on... what?

 

One millisecond is a very long time in electrical terms.

But a spark plug is getting a high voltage for a fraction of a millisecond, just like you want.

 

Given the area of application described by the TS, I suggest researching medical publications under the topic of "Pulsed Field Ablation."
Those are articles and papers written by folks who know about that stuff. Also, there are suppliers of equipment who actually know what they are doing. The TS application seems rather more unwholesome.
