EQUALS is a BAD IDEA. You need some amout of hysteresis.

<2 V
between 2 and 6 V
Between 6 and 10
>10 V

It's an over the top product, so inputs can exceed the comparitor supply voltage by a lot.

 

You can also build a 2-bit ADC with fewer components than four separate analog comparator circuits.
(The schematics shows LM339 but you can use a single comparator IC. If all you have is LM339 then you may just as well use the 4-comparator circuit already given.)

 

What I outlined can be two LTC6700-? series chips.
