Need to raise data level from 5Vpp to 40Vpp. The pulse is 1microsecond duration and repeats every2 microseconds. The optocouplers and opamps looked at do not exceed 30Vcc. Suggestions?

 

btw, 1μs square pulse every 2μs is a 500kHz square wave.

Use a transistor.

A single transistor will invert your signal, a second will flip it back.

 

If you need more than one level converters consider getting ULN2003.

The Rb2(100K) maybe too high for 500Khz.
