I am looking for a source for wall adapters that bring out the earth ground
connection in the output cable. I require new adapters (not surplus)
with agency approvals. The vast majority of wall adapters do not
bring out the earth ground connection.

For an output voltage I can use either 5-12V AC, 5-15VDC,
or a dual DC supply 5-15V. A current from 200mA to 500mA
is fine. For a DC supply connecting the earth ground to the common
is fine.

Are you needing quantity, or would surplus (one of a kind) work?

Don't know about the States, but over here many commercial AV suppliers source these as AV equipment often has a separate earthing terminal on the back.

(Perhaps this is even your intended application)
