Timer Circuit - Blew Up

Hi, I've designed this circuit. It's for simple switching fan in bathroom for some time then shut it off. I tested it today and there was bright blue spark. Circuit breakers did'nt break up so there wasn't major short or something. I wanna know if anybody sees major error in this schematic so I could work it up a little. Thank you all!
