R5 Through R9, how do I know which SMD to buy? Can I get any as long as its 22.1 ohms?

The sort of resistor will depend on the package size the PCB is designed for. I generally use 1206 sizer parts as they are easy to handle.

And for this application, the value is not real critical. You may fond 22R easier to get.
This one may be better as you can see how the packages go named...

Have you checked the FETs for gate shorts? If a resistor is open, then something else may have caused it.

 

Yes, had a hot to ground issue.
