Just some things to scope at if you have the time.
U2 -3 and 6.
U3 - 1, 3, & 6
Lower R14
What does it drive?

Do you have a 1000uF or 3300 uF capacitor for power supply?

What power supply are you using? make/model number/current rating/

one amp LED will suck down the power supply if by-pass capacitors are not big.

Also, your load is 1 amp and your Op Amp is limited to 1 amp. A little droop may be expected with such a close match?
