It's a 25 hp Hawker Siddeley 3 phase ac motor with 12 leads. I found the proper wiring diagram from a guy in Korea but the motor leads are labelled 6,6,7,7,8,8,T6,T6,T7,T7,T8 and T8.

The like leads are twisted together at the end, so there's 6 pairs.

Doesn't follow the typical 1-12 convention.

On each matching pair there should be a second indicator on one label (line or dot over under or beside one number set on one lead) that designates which end of the winding it is.

Once you have that you can relabel each winding end with the more standard code you are familiar with.

 

its probably a dual speed or dual voltage motor, so it will be wired in Star or Delta , like this type
