Looks OK to me too!

 

The title refers to a PIC16F828 but post #3 shows a PIC16F628. which is correct ? (I can't find any data for a PIC16F828 but I have used many PIC628s.)

Do you mean 1828 ?

 

Looking at post #3 I think you have program data and program clock the wrong way round.
Pin 12 is program clock and pin 13 is program data.

Yes it appears Les is right, Pickit4 pin 4 is Data(PGD) pin 13 . pin 5 is clock (PGC) pin 12.
