Need help identifying SMD component

I am trying to identify a part on a board that has gone bad. I believe this to be a thermistor (RT - resistor, temperature) and may be current limiting. It has only two contacts. I was unable to find anything on google about this part. Any help in identifying either the manufacturer or the part specs would be greatly appreciated. Here is the link to the pic.

How did you get the picture?

I've tried to take pics of several components, with several cameras, and no luck.

Is it taken through a magnifying glass?

Definitely a polyswitch. Thats a Tyco Electronics logo on the right, looks like an upside down V over another V. Check the spec sheet on Tyco's website for what the 5.0 means. In the TE polyswitches I've encountered its a current rating.
