I haven’t calculated the gain but both IC have + and - reversed. What chips are you using?

I dont know I've designed it on mutism online, and there is not specified what kind chips so I choose 3 channel amp

 

I am guessing that you copied the schematic of a Multiple Bandpass Filter wrongly:
1) The inverting and non-inverting inputs are swapped.
2) Your input is 12VDC instead of a 1915Hz signal.
3) Your opamps are missing a power supply.
