Help with photodiode amplifier design

In the above image, I've drawn out a schematic for a photodiode amplifier. I would like to know how to calculate V_out/I_in, the values of R4, R5, and R6, and how to minimise distortion as well.

What are you using to "drive" the diode, what are you driving, and what is the frequency range of the input?

The diode is simply reverse-biased across the 5V supply

--> What are you driving?

The next stage will be a sub-Bessel (Q=0.500) 2nd-order lowpass filter at 100 kHz. Gain of the filter stage is 1.

Bandwidth is 10 MHz.
