Circuits Lab Help with coding a circuit in HDL

I have been having trouble with the following task in my Circuits Lab class. I have attempted multiple times but keep getting an error code when trying to simulate the code. Specifically, I get the error "C0050: Type Mismatch". Attached is my attempt to code the given situation, as well as the question itself with the corresponding Truth Table and Schematic.
