Tattoo machine triple tap

I started my first real electronics project today. I am building a device that will allow me to run three tattoo machines through it. The layout is as follows:

1/4" phone plug is power in from my power supply, off of this i have 3 wires soldered to the + lead that will take power to the constant power lead of 3 SPDT mini toggles. One switched lead will not be used. The other switched lead will send power to a blue 5mm LED through a 150 ohm resistor, as well as another 1/4" phone plug for machine hookup. I tested the project just now and I get power through the switch to my machine. However, the switch does not interrupt power and the LED does not light when power is on at all.

Can anyone assist?

Each machine runs on variable voltage. The power supply itself generates between 1.5v - 18v DC. However, typical operating voltage can be as low as 5.5v but as high as 12.
