I want to find phase shift between A and B output (and also A-C & A-D)
I use EWB to find phase shift but I need Formula for phase shift and also gain of A-B (not in dB)(A-C & A-D) (Gain VB/VA VC/VA VD/VA)
Any one can help me to find formula of gain and shift phase?

You will have to solve the network. Have you studied the various methods for solving circuits, such as the mesh and nodal methods?

What effort have you made so far to solve the problem other than using EWB?

I know KVL(mesh) and KCL(Nodal). but I can not find gain(loss) I don't know why? I think there is simple formula for this job!
Just tell how can I find Voltage of A and B I can do it for other nodes.

Get rid of the opamp, etc., and just keep the 3 CR sections. Apply a 1 volt source to the input (left end) and solve the network for the voltages at the other nodes. Then take the ratio of VB/VA and you have the gain for that pair of nodes. Solve the rest in a similar fashion.

Show your work if you still have a problem

Have a look at this for some ideas (the C's and R's are swapped, but you should be able to figure out how to deal with that):
