help making table representing programmed rom chip.

    I need to make a table that represents a "programmed" ROM chip.

    The four bit binary number 1011 = 11 is squared and converted to an 8 bit number. 11^2= 01111001 = 121. There are 16x8 ROM available. can someone explain where the values of the table come from. The only part I understand is the 0 to 15 and the D7 to D0. I do not under stand which rows get the data, and of those rows that do, which columns get a 1 or 0.
