Hi Jeremy,

I realize this post is more than a year old, but I'll answer so you or someone else might benefit.

From the pictures, it appears that you've only connected higher order address lines, but haven't inserted the adapter in the ZIF socket. If this is the case, that's the problem.

Another issue is that you have VCC set to 5V, but it should be set to 6.2V for proper program margin checking. The EPROM would program at the lower VCC, but not reliably.

You have your programmer set to PCB4.5 compatibility, so you need to be using an appropriate version of the software; probably 0.98D5 or later. Or a most of the bits won't be programmed.
