ancient opamp

Our equipment uses an opamp (buffer technically I guess), called an Intech A-401. Our datasheets are pretty lacking. It is able to source 100mA, has gain roughly 1, 30MHz bandwidth, slew rate 1000V/uSec. But I don't know much else, and these aren't made anymore. They are the potted module type from the early 60's and 70's. (see attached)

Does anyone have any information about this device? Datasheets, catalogs, what happened to this company?
