Wiring up a 4 pole double throw switch?

Hmm, i went to buy a double throw switch from maplin today - only to realise i got a 4 pole version...

Can somebody help me out how to wire it up? I have a 9v input, then it should switch either to a circuit with 4 LEDs, or 2 UV LEDs.
