If someone could please help I need to hook up a 12 v dc motor to go in constant forward then reverse off of two limit switches i have dpdt limit switches are these even the right type?

its a 12vdc 4 rpm very small prob. 30mm dia. it needs to go back and forth propetually i have a 12v 3 amp power supply

 

thank you that looks like what i need but what type of relays and are the limit switches just standard ones please specify what type of relays and switches i need and what are the other switches in the diagram?
