integrating ADC

So I am trying to design an integrating ADC and I am having trouble with the integrator part. The output of the integrator is not very accurate, for example, when the output should become 0 at 465us. It becomes 0 at 464us.
The op-amp that I have designed for the integrator is a telescopic op-amp.
I have two questions,

1. How to improve the accuracy?
2. When designing an integrating ADC, is it a must to use switch capacitor integrator? When I use switch capacitor integrator, the charge injection from the switch would change the result...

Is this a school exercise? There are so many on the market, that it is a lot like reinventing the wheel.

Is the integration dual- or single-slope?

What is a "telescopic" op amp? I have never heard the term before.
