How to identify a ceiling fan capacitor

I'm trying to fix a ceiling fan for someone, and it has a 5 wire capacitor, which I cannot find, because I don't know exactly what type it is.

It's a Lowes Harbor Breeze ceiling fan, but they don't have any parts, except the switch.

The numbers on the capacitor are C61 E185116
It lists three outputs of 4.7 uf +- 5%
6 uf +- 5%
6 uf +- 5%

I looked on Mouser web site, and they list the following capacitor types
Aluminum...
Aluminum...
Feed Through
Film...
Niobium Oxide
Super...
Tantalum
Trimmer/Variable

Are any of these the type I need?

When I had a problem with a Harbor Breeze fan, I called the number on the literature shipped with the fan (the manufacturer, not Lowe's,) and they sent the parts I needed at no charge.

After several dead ends, I was finally able to find out the name of the fan manufacturer.

Lowes cusomer service, for Harbor Breeze fans, (1-800-643-0067, option 2) gave me the name and phone number of the actual fan manufacturer.

It is Lytex Industries, Phone # 1-800-527-1292

I didn't have the fan model #, but they were able to ID the capacitor by the number of wires, and the colors of the wires.

They are going to send me a replacement capacitor for free, it will take 7-10 days.
