digital feedback controller using pole-placement method

hi
i'm currently trying to design real-time temperature control of process trainer. I need to design using matlab simulink. I had find the transfer function, the data for the controller and the sampling time. but i still don't get the result. I think my model is wrong. Anybody can help me?
